Kidney transplantation is considered one of the most advanced therapeutic options for patients with chronic kidney failure. However, it is not limited to the surgical procedure alone; rather, it represents an integrated medical journey that begins with a comprehensive set of laboratory, immunological, and functional investigations. These tests aim to ensure the readiness of both the recipient and the donor, and to minimize the risk of rejection and post-transplant complications. 🧪 First: Immunological and Tissue Compatibility Tests These investigations represent the cornerstone of successful kidney transplantation and include: ABO Typing Determination of blood group (A, B, AB, O) for both recipient and donor. HLA Typing (Human Leukocyte Antigen) Assessment of leukocyte antigens to reduce the risk of immune rejection. Crossmatch Test Evaluation of the interaction between the recipient’s serum and donor cells to detect cytotoxic antibodies. PRA (Panel Reactive Antibody) Measurement of the percentage of pre-existing antibodies in the recipient. DSA (Donor-Specific Antibodies) Detection of antibodies specifically directed against the donor antigens. 🧬 Second: General Blood Tests These tests aim to assess the overall health status of the patient: CBC: Complete Blood Count RFT: Renal Function Tests (Creatinine, Urea, Na, K, Ca, Ph) LFT: Liver Function Tests (ALT, AST, ALP, Bilirubin, Albumin) PT / INR / aPTT: Coagulation profile FBS & HbA1c: Assessment of diabetes mellitus Lipid Profile: Blood lipid levels TSH / T4: Thyroid function tests (when indicated) 🦠 Third: Infectious Disease Screening Performed to detect infectious diseases that may pose serious risks after transplantation: HBsAg: Hepatitis B virus Anti-HCV: Hepatitis C virus HIV Test TB Test: (Quantiferon or Mantoux) VDRL: Syphilis screening CMV & EBV: Common post-transplant viral infections Urine Analysis & Culture ❤️ Fourth: Cardiac Evaluation To assess the heart’s ability to tolerate surgery: ECG: Electrocardiography Echocardiography Stress Test: When coronary artery disease is suspected 🌬️ Fifth: Respiratory System Assessment Chest X-ray (CXR) Pulmonary Function Tests (PFTs) Chest CT Scan: In cases of chronic pulmonary disease 🧍‍♀️ Sixth: Additional Important Investigations CT Abdomen & Pelvis: Evaluation of vascular anatomy Dental Check-up: Detection of potential infectious foci Pap Smear & Ultrasound: Gynecological screening Psychosocial Evaluation: Psychological and social assessment to ensure treatment adherence ✅ Conclusion Kidney transplantation is not merely a surgical intervention, but a comprehensive medical system that includes immunological, clinical, and psychological evaluations. These investigations are designed to protect both the recipient and the donor and to ensure the short- and long-term success of the transplant. Adherence to these pre-transplant assessments represents the most crucial step toward a healthier and better quality of life after transplantation.
