Introduction<br />Diplopia, or double vision, occurs as a result of a variety of factors, including eye muscle weakness or specific ocular and systemic conditions. This article reviews the main causes and evidence-based treatment methods used to manage diplopia effectively.<br />1. Ocular Structure and Anatomical Causes of Diplopia<br />Double vision may result from anatomical misalignment of the eyes or refractive disorders. Common causes include:<br />• Astigmatism, strabismus, and keratoconus, all of which can disrupt the alignment of visual input from each eye.<br />• Post-refractive surgeries, such as LASIK, may occasionally result in visual disturbances if lens positioning or corneal reshaping is imperfect.<br />2. Dry Eye Syndrome<br />Insufficient lubrication of the eye surface can lead to blurred or double vision. Inflammatory conditions or allergic conjunctivitis can worsen dryness and visual instability, emphasizing the importance of a stable tear film for proper vision.<br />3. Eye Muscle Weakness<br />Extraocular muscles play a vital role in coordinating binocular vision. Muscle weakness, as seen in conditions like myasthenia gravis, impairs the ability of the eyes to move together, causing misalignment and resulting in diplopia.<br />Some individuals may also experience transient diplopia following certain ocular surgeries, particularly those involving muscle manipulation.<br />4. Cataracts<br />The most common lens-related cause of double vision is cataracts, characterized by clouding of the natural lens. These lenses are custom-designed to realign images or block one of the overlapping images, thereby restoring visual clarity and reducing strain.<br />2. Physical Eye Exercises<br />For certain patients, orthoptic exercises can be effective. These therapeutic eye movements aim to strengthen weak eye muscles, enhance coordination, and gradually improve focus—leading to a reduction in diplopia symptoms over time.<br />3. Medical and Surgical Interventions<br />In post-surgical diplopia—particularly after strabismus surgery—additional corrective surgery or targeted eye exercises may be necessary to resolve residual misalignment.
