Its type of anesthesthetic technique<br />by which a mild dose of sedation is<br />applied to induce anxiolysis(anxiety<br />relief ) , hypnosis , anterograde<br />amnesia (in ability to form new<br />memories ) . The patient is not <br />unconscious but sedated during<br />surgery or other medical<br />procedures, like dental procedures<br />The patient called as on twilight<br />state ,where patient is sleepy ,relax<br />&able to follow simple directions by<br />the doctor. The technique is depend<br />on anesthesiologist experience , it<br />could be : oral temazepam ;<br />inhalation of N2O ; intravenous <br />midazolam , narcotic analgesic<br />fentanyl good choice , usually more<br />medicines used in twilight<br />anesthesia not offer good analgesia<br />so combined with regional or local<br />anesthesia . Any level of sedation<br />need assisted ventilation by ETT or<br />LM it considered not do the trick as<br />twilight anesthesia .<br />Why we headed to twilight<br />anesthesia ?<br />Fast recovery , patient remain<br />conscious , controlled breathing by<br />own , obey orders , his feeling like<br />light head then no any memories . It<br />is good choice for short modern<br />cosmetic surgeries & dental<br />procedures ; less cost & good choice <br />for those with serious medical<br />problems who's going on short<br />procedure than general anesthesia .<br />Ketamine at it's sub anesthetic dose<br />is good choice for twilight<br />anesthesia ,as sedative & analgesic .<br />Note that twilight anesthesia need<br />monitoring ; but not that so close<br />that make the surgeon more<br />comfort & relax during procedure .<br />Pearl : twilight anesthesia is<br />relaxing & comfort the surgeon &<br />his patient .<br />The end<br />By dr.
