Fissured Tongue
Fissured tongue, also known as scrotal tongue, is a common benign condition characterized by the presence of grooves or fissures on the dorsal surface of the tongue. Although typically asymptomatic, fissured tongue may occasionally cause discomfort, especially when debris or food particles become trapped within the fissures. This condition is often noted incidentally during routine dental or medical examinations and does not usually require treatment unless complications arise.
Etiology and Pathogenesis
The exact cause of fissured tongue is not entirely understood, but it is thought to have both genetic and environmental factors contributing to its development.
1. Genetic Predisposition:
Fissured tongue is often inherited in an autosomal dominant pattern. It is more commonly observed in families with a history of the condition.
2. Associated Conditions:
o Geographic Tongue: Fissured tongue often coexists with geographic tongue, a condition characterized by areas of desquamation on the tongue’s surface.
o Down Syndrome: Fissured tongue is a common finding in individuals with Down syndrome.
o Melkersson-Rosenthal Syndrome: This rare neurological disorder, characterized by recurrent facial swelling, facial palsy, and fissured tongue, includes fissured tongue as one of its hallmark features.
o Aging: The prevalence of fissured tongue tends to increase with age, possibly due to wear and tear on the tongue's surface.

1. Appearance:
o Fissured tongue is marked by multiple grooves or furrows on the dorsal (upper) surface of the tongue.
o The fissures vary in depth and length, often radiating from the midline.
o The most common pattern involves a central groove running along the midline with branching lateral fissures.
2. Symptoms:
o The condition is generally asymptomatic.
o Rarely, patients may report burning sensations, sensitivity, or mild pain if food particles or debris accumulate in the fissures, leading to irritation.
3. Prevalence:
o Fissured tongue affects both genders equally and can appear at any age, though it is more commonly noticed in adults.

Prognosis
Fissured tongue is a lifelong condition with an excellent prognosis. It does not predispose individuals to malignant transformation or other serious oral diseases. The condition may become more pronounced with age but remains benign and manageable with proper care.
________________________________________
Conclusion
Fissured tongue is a benign and often asymptomatic condition characterized by grooves or fissures on the dorsal tongue surface. While its exact etiology remains unclear, it is frequently associated with genetic predisposition and certain syndromes. Diagnosis is straightforward, requiring only a clinical examination, and management focuses on maintaining good oral hygiene to prevent complications. Patient reassurance about the benign nature of fissured tongue is key to addressing any concerns regarding this common anatomical variation
Ass.Lec.
