It is considered a normal anatomical variation, typically asymptomatic, and has no pathological significance. Despite its benign nature, its distinctive appearance can sometimes be mistaken for other oral lesions, necessitating accurate identification to avoid unnecessary intervention.<br />Etiology and Epidemiology<br />The exact cause of leukoedema is not fully understood, but it is thought to result from an accumulation of fluid within epithelial cells, leading to intracellular edema. This creates the characteristic opalescent appearance.<br />Leukoedema is more common in certain populations, particularly among individuals with darker skin tones. Its prevalence is higher in smokers, suggesting that smoking may contribute to its development or exacerbate its appearance.<br />________________________________________<br />Leukoedema typically presents as:<br />• A diffuse, grayish-white, or milky appearance on the buccal mucosa (the inner cheeks).<br />• A smooth or slightly wrinkled texture, which may become more pronounced when the tissue is stretched.<br />• Symmetrical involvement, often bilateral.<br />The condition is most commonly observed in the buccal mucosa but can occasionally extend to other oral mucosal sites, such as the labial mucosa or the floor of the mouth.<br />A key diagnostic feature is its reversible nature: when the affected area is stretched, the grayish appearance diminishes or disappears, confirming the diagnosis.<br />________________________________________<br />Differential Diagnosis<br />Leukoedema should be differentiated from other white lesions of the oral cavity, including:<br />• Leukoplakia: A precancerous lesion with a persistent white patch that does not disappear when stretched.<br />• Lichen Planus: A chronic inflammatory condition with a characteristic reticular or lace-like pattern.<br />• Candidiasis: A fungal infection that presents as white patches, which can often be wiped off, leaving an erythematous surface.<br />Diagnosis<br />Leukoedema is typically diagnosed clinically based on its characteristic appearance and the blanching effect observed when the mucosa is stretched. No additional diagnostic tests or biopsies are usually necessary unless the presentation is atypical or there is concern for another condition.<br />________________________________________<br />Management<br />Leukoedema does not require treatment as it is a benign, asymptomatic condition with no risk of malignant transformation.
